The Admissions Team of St. Patrick’s Home is here to help the applicant and their loved ones through the admission process. Please call the Admissions Department at (718) 519-2800 or fill out the form on this page. Our Admissions Department looks forward to discussing any questions you and your family may have about our Short-term Rehabilitation or Long-term Care Services.

In order to be considered for placement, an application and Patient Review Instrument (PRI) must be completed. If you or your family member are in the Hospital, please ask the discharge planner to fax the PRI directly to St. Patrick’s Home for review. If you or your family member are currently at home, the PRI can be completed by a Home Care Agency / Registered Nurse.
